Why They Need a Check Even If They Don't Hurt

You brush before leaving home and notice a red streak in the foam. Since it doesn’t hurt, you may think it is enough to brush more gently there or wait for your next cleaning. Bleeding gums deserve a check-up: noticing when it happens helps describe the problem, but the color of the foam cannot tell you what treatment you need.

Bleeding is a sign, not a complete diagnosis

Inflamed gums can bleed when you brush, floss or bite into something hard. The American Academy of Periodontology warns that gum disease symptoms are often silent and sometimes do not appear until advanced stages, so waiting for pain can delay an evaluation. It is also worth reporting redness or swelling, persistent bad breath, receding gums, loose teeth or changes in your bite (American Academy of Periodontology, n.d.).

Not all blood in the mouth has the same cause. Describe whether it comes from a specific area, happens spontaneously or started after changing your routine. Bring a list of medications, conditions and relevant history. The US National Institute of Dental and Craniofacial Research (NIDCR) includes medical history in the evaluation and identifies smoking as the most significant risk factor, along with diabetes, age, genetics, hormonal changes and some illnesses and medications (National Institute of Dental and Craniofacial Research, 2026).

Gingivitis and periodontitis: what changes

Plaque is a film that contains microorganisms. When it builds up, it can contribute to an inflammatory response in the gum. If it is not removed daily, it hardens into tartar, which can only be removed by a professional cleaning; brushing does not remove that deposit (National Institute of Dental and Craniofacial Research, 2026).

Plaque-induced gingivitis affects the gum and can be reversed with professional treatment and proper daily care. Periodontitis involves loss of the support that attaches the tooth to its tissues and can include bone destruction. They are not interchangeable names, and they cannot be told apart by how much you bleed at home (American Academy of Periodontology, n.d.; Heitz-Mayfield, 2024).

This difference matters because reducing inflammation and evaluating damaged support are related but different goals. Less bleeding does not, by itself, prove that lost support has been regained. The evaluation must identify which tissues are affected (Heitz-Mayfield, 2024).

What the professional checks

The exam may include a probe to measure the spaces around the teeth, recording bleeding on probing and assessing the attachment level of the tissues; depth alone does not show whether support has been lost. When there are signs of periodontitis, X-rays show how much bone has been lost. A 2024 diagnostic review stresses that these measurements must be interpreted together: a single depth reading is not enough to distinguish gingivitis, periodontitis and other conditions (Heitz-Mayfield, 2024).

You can ask what your gum chart means and whether loss of support was found. Ask for an explanation of the affected areas and how the response will be checked. You don’t need to memorize numbers to understand the reason for the plan.

Treatment and follow-up go together

For stage I to III periodontitis, the EFP guideline proposes a sequence: improve hygiene and control risk factors, provide professional treatment of deposits above and below the gum, re-evaluate and decide whether further steps are needed. Then maintenance care is established. Whether each stage is needed depends on the findings and the response (Sanz et al., 2020).

Ask to be shown a between-the-teeth cleaning technique you can do and explain what makes it hard to keep up. If an area bleeds, ask how to care for it without hurting yourself; stopping cleaning does not solve the cause of the inflammation. Brushing and interdental cleaning are part of periodontal care (National Institute of Dental and Craniofacial Research, 2026).

At the end, look for three answers: what the diagnosis is, what you will do and when the result will be checked. Knowing them turns a sign you may have gotten used to into a concrete plan to care for your gums and the teeth they support.

Want us to review your case? At The Special Teeth we see patients in English and Spanish, about 10 minutes from the San Ysidro border crossing, and we accept Aetna and MetLife PPO dental insurance.
