The Stages to Know Before You Start

You want to replace a tooth and start thinking about time off work, transportation and dates. Then the question comes up: how long does a dental implant take? To answer it, you have to separate the surgery appointment, the integration with the bone and the making of the tooth you will actually use.

Treatment is usually organized over several months

As a guide, the Leeds Teaching Hospitals leaflet states that implants are left to integrate, meaning bond with the bone, for two to six months before they are used. That interval corresponds to one phase of treatment; it is not a guaranteed date for receiving the final restoration for a specific patient (Durey, 2025).

To plan ahead, ask about the total duration and each wait between appointments. An estimated surgery date does not tell you when the process will end.

Stage one: evaluate and prepare the site

The evaluation includes examining your mouth and obtaining the records needed to plan the position of the implant and the restoration. Other procedures may be needed before surgery; the proposal should identify which ones and how they affect the timeline (Guy’s and St Thomas’ NHS Foundation Trust, 2024).

If you need an extraction, ask how it will be coordinated with the implant. In ITI terminology, immediate placement means inserting the implant on the same day as the extraction (Gallucci et al., 2018).

Stage two: place the implant and monitor healing

During surgery, under local anesthesia, the gum is lifted, the implant is placed in the bone and the wound is closed; Guy’s and St Thomas’ Hospital removes the stitches about a week later and usually waits at least three months before the implant supports teeth. Some implants are covered by the gum and need another small procedure to expose them before the restoration (Guy’s and St Thomas’ NHS Foundation Trust, 2024).

The gum may heal in one or two weeks, while the bond between implant and bone takes longer. So the area looking fine on the outside does not confirm that it is ready to support the tooth (Durey, 2025).

Check-ups help decide when to move forward. Ask what progress they expect to see and which instructions you should follow in the meantime.

Stage three: prepare and adjust the restoration

The crown, bridge or prosthesis requires records and fit checks. This stage can include several appointments (Guy’s and St Thomas’ NHS Foundation Trust, 2024). A temporary restoration is often placed first, which helps shape the gum around the implant and check the tooth’s form before the final one is made (Durey, 2025).

Clarify what you will leave each appointment with, whether you can chew with it and what restrictions it will have. A temporary also needs care instructions.

What “immediate loading” means

In the ITI definitions, immediate loading connects a prosthesis in contact with the opposite arch within the first week. Early loading happens between one week and two months; conventional loading waits more than two months. These are protocol categories, not promises of a final prosthesis in one day (Gallucci et al., 2018).

The 2023 ITI consensus addressed one specific scenario: a single tooth in the front of the upper jaw. It concluded that placing and loading the implant immediately can be reliable when conditions are favorable, although surgical, technical and biological complications can occur. That scope matters: its conclusions should not be automatically applied to any molar or full-mouth rehabilitation (Morton et al., 2023).

What can lengthen or change the plan

A prior graft can lengthen treatment by four to six months, according to Guy’s and St Thomas’; in other cases, a minor graft is done during the same implant surgery (Guy’s and St Thomas’ NHS Foundation Trust, 2024).

Initial stability and patient or site conditions can change the protocol. The ITI recommends that, from planning and consent onward, alternatives be in place in case the criteria are not met during surgery, for example implant stability (Gallucci et al., 2018).

Ask for a timeline that distinguishes estimates from confirmed appointments. If you are traveling for treatment, ask how many visits might be needed, who will do the check-ups and what would happen if a stage has to change.

A useful answer might sound like this: “First we’ll prepare this area, then we’ll confirm integration and finally we’ll adjust the restoration.” Understanding those stages lets you plan with clear expectations and keep up with follow-up.

Want us to review your case? At The Special Teeth we see patients in English and Spanish, about 10 minutes from the San Ysidro border crossing, and we accept Aetna and MetLife PPO dental insurance.
