It extends into canals inside the roots. When indicated, root canal treatment removes inflamed or infected pulp, cleans that space and seals it. Canal number and shape vary between teeth.

Treatment involves more than removing a nerve. Root anatomy may include curves, branches and canals that are difficult to locate. Diagnosis, isolation and disinfection therefore matter as much as the material used to fill the canals.

Background: from pain relief to infection control

In 1965, Kakehashi, Stanley and Fitzgerald compared exposed dental pulps in germ-free rats and animals with their usual microorganisms. Their work helped establish the role of microbes in disease of the pulp and tissues around the root. This was an animal experiment, not a prediction of an individual patient’s outcome. [4]

Modern endodontics combines that biological foundation with clinical diagnosis, sealing materials and restoration. The European Society of Endodontology’s 2023 S3 guideline organizes decisions about pulpal and apical disease around the evidence and the patient’s circumstances. [3]

When might you need treatment, and how is it diagnosed?

Deep decay, trauma, a crack or problems around previous restorations can affect the pulp. The clinician reviews your pain history, tests sensitivity and percussion, examines the gums, bite and possible cracks, and takes appropriate radiographs. Shapes and materials are simplified for education.

Many root canals can be completed in one or two appointments; anatomy, retreatment or individual needs can change the schedule. Buildup and crown appointments are a separate stage. Before traveling from the United States, confirm the full plan rather than booking only the root canal visit. [2]

Pain, recovery and aftercare

Local anesthesia helps control discomfort during treatment. Tell the clinician if you feel pain so it can be addressed. Tenderness on biting may occur for the first few days; contact the office if pain worsens, becomes severe or persists. Follow your medication and hygiene instructions, and avoid overloading the tooth before its permanent restoration is in place. [2]

Benefits of saving a tooth that can be restored

Treating a tooth with a reasonable prognosis can relieve pulpal pain, control infection and retain its root, position and periodontal ligament, which contributes to bite sensation. It may also avoid extraction surgery and the stages required to replace the tooth. This does not mean every tooth should be retained regardless of prognosis. [22]

How successful is treatment?

A 2022 systematic review included 42 studies of primary root canal treatment. It estimated 82% success under strict healing criteria and 92.6% under broader criteria that allow a lesion that is shrinking. Most studies provided low-quality evidence, and results differed among studies. These figures describe groups of patients: they neither guarantee an individual result nor represent The Special Teeth’s success rate. [5]

“Success” and “survival” are also different outcomes: a tooth may remain in the mouth while needing further treatment. Follow-up considers symptoms, function and imaging, alongside the restoration and the health of the supporting tissues.

The final restoration belongs in the treatment plan

A tooth may already have lost structure to decay, fractures or previous restorations. Root canal treatment does not replace that structure. A weakened molar often needs a crown or another restoration that protects its cusps; the design depends on the remaining tooth and the bite. A post is used only when needed to retain a buildup: it is not mandatory and does not make a root unbreakable. [2]

1, internal canal treatment; 2, one possible external restoration using a crown.
